To Apply For A First Learner Permit You Need The Following:
- A completed application form D.201- All questions must be answered on a First Application.
- A theory certificate in the category of vehicle (eg: B Car, C Truck) you are applying for learner permit
- Original Irish or British Birth Certificate or Passport (Non EU Passport with current Visa or cert of registration) National Identity Cards accepted as per RSA.
- Original Marriage Certificate (if appropriate)
- A Medical Report form D.501 for classes C, C1, D, D1, EC, EC1, ED and ED1
- A medical report D.501 is also required if you suffer from any of the Diseases or Disabilities listed on page 2 of D.201.
- An eyesight report (form D.502) for classes A, A1, B, EB and W - eyesight report valid for one month from Examination.
- Fee - €15
- Two identical passport photographs (signed on back by applicant) - 3.5cm x 4.5cm.
- Please forward a seperate sticky label with signature if using downloaded application form or photocopied form.
- Any learner permit with a start date on or after the 4th April 2011 must complete the Essential Driver Training before applying for the driving test. Details of this can be found on the RSA website www.rsa.ie
To Apply For A Second Learner Permit You Need The Following:
- Application Form D.201
- Two identical passport photographs (signed on back by applicant)
- Old Learner Permit (if previous Learner Permit is not available a lost license declaration D8A stamped at the Garda Station is required) In this case you must also forward an original form of photographic identity i.e. passport or cert or registration or national identity card.
- Fee of €15

To Apply For A Third Learner Permit You Need The Following:
- Application Form D.201
- Two identical passport photographs (signed on back by applicant)
- Failure Certificate (valid for two years from date of issue) / acknowledgement letter confirming you have applied for driving test or test date letter in category you want learner permit in. If you are forwarding email of confirmation please supply email with category of vehicle eg B for car - reference no. on its own is not acceptable.
- Old Learner Permit (if Learner Permit is not available a lost license declaration form D8A stamped at the Garda Station is required) In this case you must supply an original form of identity e.g. passport, cert of registration or national identity card.
- Fee of €15
- Please supply a sticky label with your signature if you are using a downloaded application form or photocopied form
To Apply For A Full Driving Licence You Need The Following:
- Application Form - D401 fully completed and signed by applicant, ensure that you have signed the label on page 3 of this form.
- Two identical passport size photographs (signed on back by applicant)
- Your current or most recent Driving Licence and/or Learner Permit (If you hold a Learner Permit in a different category this also must be submitted if issued in the last 5 years). In a case where this licence has been lost, mislaid or stolen please submit a D8A form stamped and signed at a Garda Station and some form of identity.
- Appropriate Fee
- 1 year licence - €5 (Medical Grounds Only)
- 3 year licence - €15
- 10 year licence - €25
Licence up to age 70
-
- (not more than 10 years) - €25
Please note there is no fee for an application to renew a Driving Licence/Learner Permit where the applicant is aged 70 or over.
And, If Required In Your Case:
- Certificate of Competency - if converting from Learner Permit to full or adding catergories to current Driving Licence ( There is no fee to add categories to a current licence where the licence is submitted)
- Medical Report for:
- categories C1, C, D1, D, EC1, EC, ED1, ED (regardless of age)
- any applicant aged 70 years and over
- If applicant has a Medical Conditions - see page 2 of D401 "Disabilities and Diseases"
N.B. The Medical Report must be valid for the total life of the driving licence being applied form and must be submitted within 1 month of the date of the Medical Examination.
- Original Marriage Certificate/Passport in order to Change Name on Driving Licence through marriage or Original Birth Certificate/Passport to revert to maiden name. Deed Poll Document where applicable. All original documents will be returned to applicant. Passports are returned by Registered Post.
An application for renewal of a Full Driving Licence shall not be made more than 4 months before the date of expiry of the current Full Driving Licence.

Duplicate Licences:
- Application Form D800
- Two passport size identical photographs (signed on back by applicant)
- Appropriate Fee of €10
- Photo ID eg: Passport, GNIB, National Identity Card.
Free-in-Lieu Licences:
- Extra category added
- Change of name/address
- Removal of endorsement
- If an error has been made in the issue of licences (e.g. incorrect details etc.)
Please note the last Driving Licence/Learner Permit must be submitted or else a fee of €10 will apply and eveidence of identity as per listing.
